About the treatment

One fixed arch, carried by implants

All-on-4 is a way of replacing a whole upper or lower arch of teeth with a fixed bridge supported by four implants. The two back implants are usually placed at an angle, which can make better use of the bone that is there. Whether it suits you depends on your bone, your gums, your bite and your general health, assessed at an examination.

Whether a temporary arch can be fitted soon after surgery depends on how stable the implants are on the day; it is never promised in advance.

All-on-4 replaces every tooth in one arch with a single fixed bridge, carried by four implants rather than one implant per tooth. Two implants are placed towards the front of the jaw and two further back. The back implants are usually tilted, so that they can be longer and anchor in bone that is there, while avoiding areas where bone is thin, such as below the sinuses in the upper jaw or near the nerve in the lower jaw.

Where the bone and the stability of the implants allow it, a temporary fixed arch may be attached soon after surgery. That is a decision made on the day from how firmly the implants sit, not something that can be promised in advance. The implants then heal into the bone over several months, and only after that is the final arch made and fitted.

Four implants carry the forces of a whole arch, so planning is careful: 3D imaging of the bone, the position of each implant, your bite and the design of the bridge are worked out together. In some cases the assessment shows that more implants, such as All-on-6, or a different approach would be more appropriate. That is discussed openly at your examination.

Step by step

How All-on-4 treatment is staged

The stages below describe the usual sequence. Your own plan may differ, and it is explained to you before anything begins.

  1. Step 01

    Examination and 3D imaging

    Your teeth, gums, bite and the bone in the whole arch are assessed, with 3D imaging to measure where implants can be placed.

  2. Step 02

    Medical history and discussion

    Your general health, medicines and smoking are reviewed, together with the alternatives, including All-on-6 and removable options.

  3. Step 03

    Treatment plan

    Implant positions, any extractions, the temporary and final arch and the staging are set out, including how travel fits around them.

  4. Step 04

    Surgery

    Any remaining teeth that cannot be kept are removed and the four implants are placed, normally under local anaesthetic, with sedation discussed if appropriate.

  5. Step 05

    Temporary arch

    Where the implants are stable enough, a temporary fixed arch is attached; otherwise another temporary solution is agreed.

  6. Step 06

    Healing and integration

    The bone heals around the implants over several months while you wear the temporary arch and follow the aftercare plan.

  7. Step 07

    Final arch and review

    Once the implants are judged stable, the final bridge is made and fitted, and regular check-ups monitor the gums, bone and bridge.

Aftercare

You will be given written instructions for your own case. The general points below give an idea of what aftercare involves.

    Swelling and discomfort

    Both are common after full-arch surgery and usually settle over the following days. Your dentist will advise on pain relief that is appropriate for you.

  2. 02

    A soft diet at first

    While the implants heal, the bite on the temporary arch is protected. Follow the instructions you are given on what and how to eat.

  3. 03

    Cleaning under the bridge

    Food collects under a fixed arch. You are shown how to clean beneath it and around each implant from the first days.

  4. 04

    Avoid smoking

    Smoking can interfere with healing and with integration of the implants, so avoiding it is strongly advised.

  5. 05

    Know when to get in touch

    Contact the clinic if bleeding, swelling or pain increases rather than settles, if the temporary arch feels loose, or if you feel unwell.

What is the difference between All-on-4 and All-on-6?

Both replace a whole arch with a fixed bridge on implants. All-on-4 uses four implants, with the back two usually angled to make use of the available bone. All-on-6 uses six, which spreads the load over more implants and is often considered where there is enough bone along the arch. Which is more appropriate depends on your bone, your bite and your general health, and is decided after 3D imaging and an examination.

Will I have teeth straight after surgery?

Often a temporary fixed arch can be attached within days of surgery, but only when the implants are stable enough on the day. If they are not, another temporary solution is agreed with you. This is explained during planning so that you know both possibilities before you travel.

Why does All-on-4 involve more than one trip?

The implants have to heal into the bone before the final arch is made, which takes several months and cannot be completed within a single stay. For patients travelling to Alanya, treatment is therefore commonly split into a surgical visit and a later visit for the final arch. The timing for your case is set out before you book flights.

Am I suitable for All-on-4?

That can only be answered after an examination with 3D imaging. Your dentist looks at the amount and quality of bone across the arch, your gums, your bite and your medical history. Photographs or existing X-rays you send in advance help start the conversation, but they do not replace an assessment in the chair.

Is the surgery painful?

Implant placement is normally carried out under local anaesthetic, so the area is numb during the procedure, and sedation can be discussed for longer procedures. Swelling and discomfort afterwards are common and are usually managed with the pain relief your dentist recommends. You receive written aftercare instructions and a way to contact the clinic.

Does smoking affect All-on-4?

Yes. Smoking is associated with poorer healing and a higher risk of problems with the gum and bone around implants. When a whole arch depends on four implants, that risk matters even more. If you smoke, your dentist will explain how it may affect your treatment and what can be done to reduce the risk.

How do I look after an All-on-4 bridge?

Daily cleaning under the bridge and around each implant is essential, usually with a soft toothbrush, interdental brushes and a water flosser or special floss. Regular professional check-ups, at which the bridge may sometimes be removed for cleaning, help keep the gums and bone healthy. Your dentist will show you which cleaning aids suit your bridge.
